The necessary documentation must be submitted in the form of original documents (which will be returned by registered mail) or of certified true copies.
Certified true copies are copies which have been endorsed on each copy to confirm their authenticity. Embassies, police, Notary Publics and the authority who has issued the document can carry out the endorsement. Copies certified by private individuals are not acceptable. The National Board of Health may in some cases require original documents.
If you submit original documents you must also submit a copy of each document. If you submit certified copies and wish to have these returned to you, you must state this by checking the box below the application form. If so, you must submit an identical set of confirmed copies i.e. you must provide a copy of the certified documents. All copies must be in A4 format and without paper clips, folders, binders etc.
Documents must be translated into English or Danish. However, documents in German, Norwegian or Swedish do not need to be translated.
The translation must be carried out by:
- The educational institution or the issuing authority or
- A state-authorized translator. Translator’s name and stamp must be legible. Should the stamp not be in Danish, English, Norwegian or Swedish a state-authorized translation of the stamp is required. The translation must be accompanied by the document in the original language sealed together by the translator. The National Board of Health may in some cases accept other translations.
Please note that the attached documents must be marked with the number indicated for each document below. For example, documentation for name, birth date and citizenship must be marked “1.”
Please submit the following documents with your application:
- Documentation of name, date of birth, citizenship (e.g. in the form of certified true copies of the relevant passport pages)
- Certificate of name change/certificate of marriage.
- If relevant, a translation of the certificate of name change/certificate of marriage.
- Curriculum Vitae with education and professional experience in chronological order.
- Proof of education/graduation.
- If relevant, translation of certificate of education or graduation into Danish or English.
- Detailed transcript of your study program from your school or university of graduation describing the scope and contents of the program of study, and examinations. Clinical and theoretical training should be indicated in hours or weeks. The description should contain information about:
- Admission requirements.
- Normal duration of the program of study.
- The courses that comprise the theoretical portion of the education and the hours/weeks in each course.
- Duration and content of each of the clinical training periods.
- The institutions and locations at which the clinical training (practicum or internships) have taken place.
- If relevant, a translation transcript or description of the education from the school of graduation.
- Translation of detailed transcript from the place of education.
- Original Certificate of Current Professional Status/Certificate of Good Standing from the qualified health authorities of the country in which you most recently worked. The certificate must include statements that assert that you have the right to work within your profession, i.e. that you have not been suspended, disqualified or prohibited from practicing. The certificate must be in English, no older than three months, and the relevant health authority must send it directly to the Danish National Board of Health. The address is:
