Non-volitional Joint Position Sense in the Anterior Cruciate Ligament Deficient Knee – a pilot study

Study Design: Repeated measures multivariate analysis of variance (MANOVA) with nesting.

Objective:
1. Determine whether a between-knee difference in non-volitional joint position sense (JPS) is present in unilateral anterior cruciate ligament (ACL) deficient individuals.
2. Investigate whether a between-knee difference in the threshold to the detection of passive movement (TTDPM) could be confirmed in ACL deficient individuals.

Background:
JPS and TTDPM are the most common clinical measures of joint proprioception. While TTDPM is known to be impaired in the ACL deficient knee, JPS is not affected in these patients during volitional attempts to match an index angle. The effect of ACL deficiency on non-volitional JPS has not previously been reported. In the present study, the effect of ACL deficiency on non-volitional JPS and TTDPM was investigated.

Methods and Measures:
13 subjects with unilateral ACL deficiency were tested bilaterally using a fiber optic goniometer. The relative real error for each knee was calculated and the between-knee discrepancy was computed.

Results:
MANOVA with nesting revealed an effect of ACL deficiency [F(13,25)=3.1993; p=0.0061] on JPS. There was a four-percent mean decrease in JPS performance in the ACL deficient knee. There was no effect of ACL deficiency on TTDPM. Conclusion: These results indicate that non-volitional joint position sense is measurably impaired in the ACL deficient knee.
